Fees & Out of Network Benefits

Out-of-network benefits:

If you have out of network benefits through your insurance provider, you may be reimbursed for your sessions at New Perspective through your insurance company.  We will provide a receipt that you may send to your insurance company, or we can submit to your insurance as a courtesy.

Questions For You to Ask Your Insurance Company

1) Do I have out-of-network Benefits that apply to billing codes: 90791 for (intake)and 90834 individual sessions and 90853 group sessions.

2) Do I have a deductible that applies to out of network appointments?

3) how much (more) do I need to spend to meet it?  

4) After meeting deductible what % will I get back? Up to what cap? (e.g. 80% up to $250)
